Description:
We provide health care specialists for every home care need (Nurses, Home Health Aides, Companions). Services are available seven days a week, 24 hours a day. A nurse is on call 24 hours a day. We provide an alternative to nursing home or long term placement. Medicare Certified Services: Professional Services include: Skilled Nursing, Physical Therapy, Occupational Therapy, Medical Social Services, and Speech Therapy. Medicare, Medicaid, Quest and other health insurance pay all or most of the cost of medically necessary skilled home care. Home visits are scheduled as needed seven days a week. The plan of care is established and regularly reviewed by patient's physician. the patient and family are involved in development and implementation of the home care plan with access to multilingual interpreters when needed. Office Hours: 8 am - 5 pm. Monday - Friday (Except Holidays) Saturday 8 am - 4:30 pm.
