Description:

The Maui County Office on Aging provides services to the Islands of Maui, Molokai, and Lanai. The emphasis on Aging in Place underlies all activities in which our office engages. Every service, program, and product referred or implemented by the Maui County Office on Aging and contracted providers serve to assist Maui County’s seniors and adults with disabilities to lead independent, fulfilled, and dignified lives in their own homes and own communities for as long as possible. Services are provided through Federal (Title III), State, and County of Maui funds. For all funding sources, one must be 60 years of age and over to qualify for services. Also, for those receiving services through the State funded Kupuna Care Program (KC), there are other requirements that must be met to qualify.
General Eligibility Requirements include: U.S. citizen or qualified alien; 60 years or older; Client is not already receiving service from another government (e.g. Medicaid) or private service (e.g. Home Health Agency) provider; Live in a residence, not an institution (intermediate care facility, skilled nursing facility, adult residential care home, hospital, or foster family); Have impairment of two or more activities of daily living (ADL)* or instrumental activities of daily living (IADL)** or significantly reduced mental capacity; and Have two or more unmet activities of daily living* or instrumental activities of daily living** need.
*Activities of daily living (ADL) are routine activities that people tend do everyday without needing assistance. There are six basic ADLs: eating, bathing, dressing, toileting, transferring (walking) and continence.
**Instrumental activities of daily living (IADLs) are the complex skills needed to successfully live independently. These skills are usually learned during the teenage years and include the following: preparing meals, shopping, managing money, using the telephone, doing housework, using public transportation, and managing/taking medications.
Home and Community Based Services include: Adult day care; Case management; Chore services; Congregate meals; Family caregiver support services (caregiver or care recipient must be 60+); Adult day care respite; Counseling; In-home respite; Information and assistance; Friendly visiting; Home-delivered meals; Homemaker services; Housing Options Counseling; Information and Assistance; Legal assistance for grandparents (55+) raising grandchildren; Legal assistance for seniors (60+); Nutrition counseling; Nutrition education; Outreach; Personal care; Telephone reassurance; Transportation; and Healthy Aging Programs (Enhance Fitness).
Enhance Fitness is an evidence based, 3 times per week exercise program that helps to improve cardiovascular fitness, strength, flexibility, and balance. This program helps build relationships among participants and creates an exercise environment that is fun and friendly. Moreover, it has been scientifically-tested and has been shown to improve health and reduce healthcare costs among regular participants.
